Although this isn't exactly a "mistake," it still always bothered me. On the cover of the VHS (I'm not sure about the DVD) box, there are several zombies with red eyes that are shown under the title. One of the zombies shown is the main character, Ben. In the movie, this main character survives many close calls until he is turned into a zombie almost at the very end of the film. If you look at the box before watching the movie, it takes away some of the "surprise" and tension.
