Guyana, the Jim Jones tradgedy - Oops, Thought They Were All Dead!
At the very end of this movie when Jim Jones has made all of his followers drink the kool aide, he is supposed to be the last one left still alive yet right before he puts a gun to his head, you clearly see a shadow go across the back of the white chair he is half sitting on. This is a biggie blooper.

For starters, the movie is entitled "Guyana Tragedy: The Story of Jim Jones". Secondly, it was determined that the gun was fired from approximately 10-12 feet away from Jones, and therefore he did not shoot himself. It is widely believed that either Carolyn or Annie, (I forget their last names) two of Jones' most loyal women, shot him before taking her own life. Annie actually left a note for whoever discovered the bodies, detailing why they took their lives and why they loved Jones so much. This is not really a slip-up; you never see where the shot came from, or that Jones had a gun in his hand, or concrete proof that he was the last one alive in the compound.
