Monty Python and the Quest for the Holy Grail - Priest W/ a Silencer, Apparently
In the scene where the monks are slapping themselves in the heads w/ the boards...after a couple of the guys in the back moves the hits his head..but the 'whack' sound is not heard. Then a millisecond later all the other monks hit themselves and get the 'whacking' noise. Since the noise was added in post, and one of the monks was off on his timing and hit himself a few seconds b4 everyone see one monk hit himself silently.

Despite the fact that this is, indeed a slip-up, I think that it was done on purpose. I mean, the 'whack' sound would not have been nearly as effective if it had not been one simultaneous sound. It wouldn't have been as funny, or effective if all you heard was 'whack..whackwhack..whack..whack..whackwhackwhack' rather than one solid 'crash'.
