Run Away Bride - Moving Lei
In the scene where Julia Roberts is sitting there being made fun of by all of her friends and family, it is in this Hawaiian like setting with leis and stuff, but anyways, after a while, Richard Gere stands up and defends Julia, and she runs outside in embarrassment, after the two are outside talking watch the Hawaiian lei on Richard Gere's neck. It moves and changes positions on his neck in every shot. It must do this about 15 or more times!

That's not really a slip-up. I live in Maryland so I know when they were making the movie, it wasn't really the best weather. It rained some as you can see from other shots like outside the church right after they kiss in front of Bob. So it could have been a combination of wind, and the fact that he was moving around quickly, turning quickly to look back at her and things like that. It just moved because of the quick motions and things he was doing.
