Teenage Mutant Ninja Turtles animated series - Identity Crisis
In one of the episodes (the one where April turned into a cat) there were two huge slip-ups.
First, when Leonardo contacts Irma via the Turtles' videophone, she sees Raphael on the screen, who still says that he's Leonardo.
Then, when the hypnotized April come to the sewer to kill Splinter, Leonardo pulls his sword out and jumps to April but when he's completed his jump he turns into a Donatello, who still holds Leo's sword.

I hate to say it, but this kind of goof happened a whole lot on this animated series. This certainly was not an isolated episode.

One mistake on your part, Leonardo has two swords, so donatello could have been holding one.

There are 4 exact reason why this is a slip up - 1. It's Leonardo who begins to run 2. It's Michaelangelo who pulls the sword from his back 3. It's Donatello seen holding the sword after 4. A split second after we see Donatello with the sword, his hands are empty and Leonardo now as the sword and is standing behind Michealangelo. This is a ridiculus slip-up, what kind of animator can't get 4 colors right? what kind of animator gets the same color wrong twice?

Jeff it couldn't have just been don with a 1 sword cause his weapon was the boa not the sword.
