When they commit Wynona Rider to the institution they send her there in a cab. She gets in and sits in the backseat on the drivers side. She lights a cigarette and then has a flash back. When she comes back from the flash back she is on the passenger's side in the backseat.
In the DVD version they have some of the cut scenes; they edited a significant portion of the cab ride out of the final theatrical release. But if I recall correctly, I think at one point Winnona is laying down in on the seat. Maybe when she sat up again she moved so she could look out the other window.

when i am in a cab, i usually move around. sometimes i just get bored and have to. others i just don't realize i'm moving. this isn't a slip-up. it would be a slip-up if she would have all a sudden been sitting in the front seat.
