This is right at the start of the film when the 3 guys are in the Antarctic. Dennis Quaid jumps across the crack in the ice clutching the ice core samples he was saving. Suddenly the ice gives way and he plunges into the crevasse. The shot then pans into the crevasse where we find him clinging onto an ice axe. Now where exactly did that come from? He does have it on his belt beforehand but there is no way he'd have had time to grab it as he fell. He certainly wasn't holding it when he fell as we see his hands empty as he falls into the crack.
