The 3rd season episode "Spock's Brain". Just before the elevator door opens and Kirk stuns Luma, a film crewmember's hand quickly flashes across the lower, right hand corner of the screen. It appears to belong to the man who holds the "clapper" in front of the camera, which records the scene and take number.
