Star Wars Episode 6: Return of the Jedi - Twice...Thats Gotta Hurt
At the beginning of the movie, where Luke gets shot by the blaster in his hand, right at the shot of Lukes hand up in the air press slow motion and you can see Luke's hand, clearly with a hole in it already, get hit by the blaster bolt.
Special Requirements:
A VCR with pause or slow-motion and the movie.
