NCIS - Forgetful Fornell
In NCIS, Tobias Fornell and Jethro Gibbs are almost 'best' friends and have a long standing relationship. Fornell even married Gibb's ex-wife.
In the Season one episode 'Yankee White', Fornell and NCIS agent Kate Todd allow Jethro Gibbs onto Airforce One as they believe him to be a local coroner's assistant. Jethro and Fornell are stood about 2 metres from each other and for some reason the episode is written so that Fornell has NO idea who Jethro Gibbs is!
He doesnt even seem know Gibbs is NCIS as he is surprised when Gibbs takes the plane, which means that this is a major mistake in plotting, or Fornell and Gibbs are both suffering from Temporary One-Episode Amnesia.

The episode "Yankee White" was the pilot episode for NCIS. The writers did not forget that Gibbs and Fornell had a long standing relationship because it had not been established that they had. Those details were added later in the series by writers who forgot that Gibbs and Fornell were strangers at the beginning of the series.

This isn't a slip up. In the pilot episode, Fornell meets Gibbs through the case on Air Force One. As the series progresses, Fornell's cases and Gibbs' cases overlap several times bringing them continuously in contact. In the beginning of the series Fornell and Gibbs are constantly at each other and they realize they are a lot alike, which strengthens their friendship. As for Fornell marrying Gibbs' ex-wife, it is never stated that they were married before the time of the episode of "Yankee White."

Well I found a slip-up in your "slip-up". In the episode "Yankee White" Kate is Secret Service not NCIS, she resigns at the end of the episode.
