In the beginning of the movie Gene Wilder is teaching a class. He becomes agitated at a student who is asking about his grandfather Dr. Frankenstein. Becoming more agitated he picks up a scalpel and stabs himself in the leg. If you look closely just before he does this, there is a square shaped form which must be some sort of padding under his pant leg.
