In the pilot episode "Children of the Gods", everyone is sitting around the briefing table as O'Neill is talking to Carter. As the camera pans or shifts to show MAJOR Kowalski, he's wearing Captain's bars on his camouflage uniform, but when he's in dress uniform, he has a Major's gold oak leaf on it.
Excellent. I'm not a pro at this, but seeing MAJ Kowalski wearing captains rank was weird; especially in a show usually quite accurate in military advising (perhaps they didn't have an adviser for the pilot.)
