The part where Det. David Mills is running after the killer and ends up in an alley, he is on the ground next to a truck and his face is bleeding. The killer then comes up next to him with the gun against his head. When he removes the gun from his face, Mills bends over. There is a little tube that is visable from the back of his head that you can see and it is providing the blood to his face.
