In the opening sequence where their going over the murder scene they put the oxygen mask on Shelly 3 times without ever taking it off first. The first time is in the apartment when she is laying on the floor. The next is when they are taking her outside on the stetcher. She then speaks and it shows her close up again and they put the mask on her once more. I don't know how anyone couldn't have caught this mistake before me!
