Go to the part where Carl is removing organs and such from the woman in the tub, and Katherine comes in and starts talking to him. You will notice that Carl is smoking a cigarette, but as they talk he gets upset he throws his cigarette down. But then, a moment later, he is smoking again, and a moment later, he has no cigarette in his hand.
