When Skywalker first goes to "Castle Land" you see the girl putting a cold compress on his head. His medallian is barely visible, so she reaches inside his shirt and pulls it out. Then he sits up with a start, and all of a sudden his medallian is back underneath his shirt again, and you can no longer see it.
