Close Encounters of the Third Kind - Richard Dreyfuss's Spotter
As Roy and Jillian are climbing up the Devils tower monument Roy looses his footing and tumbles down a steep slope, if you look carefully just in front of the rock that Jillian is behind there is a stagehand wearing a head to toe black body stocking(to make him blend into the shadow of the rock) "spotting" for Richard Dreyfuss. You can see the stagehand crouching down and turning his head to watch Richard tumble down the slope

I have since discovered that this slip-up can only be seen clearly when watching certain television broadcasts of this movie. Sometimes a network or cable station will use a special "bright" print of the movie to make the dark scenes more visible on TV, this can sometimes lead to problems (Like the faint misshapen boxes that appeared around the spaceships in early network broadcasts of Star Wars). In these prints of the movie the stage-hand can be seen quite clearly crouching down and turning his head to watch Richard tumble down the slope. This is not quite as visible on the VHS version of the movie.
