Vertical Limit - Swipe of the Rotor Blade
When the rescue team goes by helicopter to as high as they can go, and the team is jumping from copter to cliff, the copter starts having turbulence and is out of control. The female member is trying to avoid the blade as it dips lower and lower, and she is backed up against the cliff. Suddenly, the tip of the blade swipes her arm. But, after the ordeal, nobody even noticed if she was hurt or not, not even the victim herself.....

Ok, why would this even count as a slip-up??? Just because the rest didn't bother to check how she was doesn't mean it was a slip-up!!...Maybe they were just too busy trying to get Peter and Montgomery Wick onto the mountain.
