In D2: The Mighty Ducks, after the team changes uniforms before the 3rd period, look at Fulton Reed (#44). Apparently, after the uniform change, he no longer has a last name. His jersey says "Fulton" on the back, even though the character's last name is Reed.
Also, in D3, his jersey sports the wrong name throughout the entire movie.
It must have been is moniker. You know Ichiro Suzuki's name on his uniform just reads Ichiro due to his fame. Maybe Hans thought everyone knew Fulton, so why not put his first name on it.
