Peter Pan (Cathy Rigby) - Doorknob on the Wrong Side
In Peter Pan starring Cathy Rigby, when they all build a house for Wendy, Peter draws on a door. However, you'll notice that she puts the doorknob on the left side of the door, when the door opens from the right.

That same thing happened in the Mary Martin version of Peter Pan, so I think that it was supposed to be intentional.
