In the end fight scene, When Maximus is driving the dagger into Commodus' throat, you'll notice he drives it in with both hands on the dagger. When the camera cuts away and returns, Maxiumus' left hand is on Commodus' cheek, in a soothing manner as he 'shush'es Commodus to death.
