Toy Story 2 - Moving Gum
In the scene when the toys cross the road, (chapter 16 on the DVD), Mr Potato-Head gets his foot stuck to a piece of gum on the road. In the first shot that we see of the pipe that falls off the lorry, we can see that he is in line with the center of the pipe, (when he reaches out of the cone). Then we see a shot of Mr Potatoe-Head pulling his foot off the gum, from inside the cone. The next shot is of the cone walking out of the way, and the pipe narrowly missing him. But now the gum and cone have moved from the center of the pipe to near the end, so he can narrowly avoid being squashed.

Actually, the pipe missing Mr. Potato-Head is not a slip-up, since the pipe rolled at an angle. However, it is a slip-up that the pipe missed him to his right because it hit a telephone pole to his left, so the pipe should have missed him to his left.
