Star Trek Voyager - Make Up Your Mind! Which Side's It Gonna Be!
To see this slip-up you must watch the eisode of voyager that aired on December 1, 1999. In this episode Starfleet Command makes contact with voyager through a microwormhole.
1. Go to the scene where Barclay is in Adm. Parris's office.
2. In the first shot when they show Tom's picture his badge is on the wrong side.
3. The next time that they show the picture Tom's badge has mysteriously repositioned itself!

I've not seen this slip-up, but is it possible that the film was reversed for some reason? that would account for the badge shifting position. Check some of the other details in the scene sometime, and try to verify that.

Just on the same point. The picture used is off the episode McNeil was in for TNG where Crusher and the cadets were trying to do the trick which got one of them killed! Hes got the Cadet Uniform on and the old compin type rounded thingy

I believe the name of the episode in question is "The First Duty", and the name of the student is Cadet Locarno. The book which I refer to is the Nitpicker's Guide for TNG, but it does not give the actor's name.

They should have kept McNeil the same character, considering the cadet from the TNG episode was a good pilot and got kicked out of Starfleet just like Tom Paris.

Is that episode called Pathfinder ? It seems a bit too 'orderly' to be a mistake - any chance it could have been done on purpose ? Just for a laugh perhaps ? :-)
